Q: What does my wellness visit include?

A: Please refer to the 2018 Enrollment Guide, and other Documents that are provided to you through your Employers. The Login information will be given to you during enrollment. These documents can be downloaded to your computer for future reference


Q: Where can I find a list of Providers?

A: We do not require you to see a Physician in a specific Network, you are free to use the provider or facility of your choice. It is important to remember that the plans are not Major Medical Insurance policies, but a Self Funded ERISA Plan provided, and paid by your employer.

When you go to see a doctor, or schedule an appointment you should tell the provider that this is a “Self-Funded plan provided through your employer.” If they have any questions they are free to call Keystone Claim Services at 888-227-5583.
