Q: What does my wellness visit include?

A: Please refer to the 2019 Enrollment Guide and other Documents that are provided to you through your Employers. The Login information will be given to you during enrollment. These documents can be downloaded to your computer for future reference.

Q: Where can I find a list of Providers?

A: Your plan uses the PHCS Network. You can find a provider in the link at the top of the page under Find A Provider. Using a PHCS Network Provider will provide you with the best discount available to you, but we do not restrict you to using only a PHCS Network. We realize that there are times when it is not possible to use a preferred provider. So you are free to use any doctor or facility of your choice. Just keep in mind that when going out of network, your discount will not be as much and getting into see a doctor may be a little more difficult.

If going out of network, please be sure to let them know that you have a “Self-Funded plan provided by your employer.” If they have any questions they are free to call Keystone Claims at 888-227-5583. The phone number will also be on the back of your Member ID Card.
